During the Italian Formula 3 series test day held at Mugello circuit on Monday 02 March 1998, the RC Motorsport entered Dallara 398 - Opel of Alfredo Melandri went off the road at the San Donato bend, at the end of the long finish-line straight. At a speed of about 250 km/h, apparently without any slowing down - probably due to brake failure - the car crashed head-on into the barrier placed beyond an about 110 meters sand bank. The nose and the cockpit of the car were completely destroyed, and also Melandri's helmet was crushed. He knocked unconscious, having sustained massive face injuries, cranial and spinal trauma and pulmonary lesions. He was taken to Florence hospital in a coma and seven days later he succumbed to his injuries.
